Our Community Outreach support service is designed to support individuals who are usually living in the family home & typically require a few hours support to help them maintain or gain their independence
We work with individuals to develop a personalised support plan that is appropriate & suitable to the individual’s needs. We consider the goals & priorities we need to focus on to achieve the desired outcomes.
